DATA SUBJECT APPLICATION FORM
General Explanation
Personal data owners, who are defined as a data subject (hereinafter referred to as “Applicant”) within Law No. 6698 on Protection of Personal Data (the “PDP Law”), are entitled to make certain requests regarding the processing of their personal data according to Article 11 of the PDP Law.
You can communicate your requests under your rights listed in Article 11 of the PDP Law to our organization, acting in its capacity as data controller, in accordance with paragraph 1 of the said Law and Article 5 of the Communiqué on the Principles and Procedures for Application to Data Controller, and in writing via this form or through other methods determined by the Personal Data Protection Board (the “Board”).
In this context, all applications in writing can be communicated to us by printing out this form, and:
- Via personal delivery by the Applicant,
- Through a notary public, or
- Via sending an e-mail to us (veriguvenligi@iloakademi.org), sent by the Applicant from the e-mail address previously indicated and registered in our system.
As per paragraph 2 of Article 13 of the Law, your applications communicated to us shall be responded to within thirty days as of their date of receipt, according to the nature of the request. Our responses will be delivered to you in writing or electronically pursuant to Article 13 of the relevant Law.

This application form was drafted to respond to your application within the due and legal period by identifying the relationship between you and our Organization and fully determining your personal data that is processed, if any. With a view to eliminating legal risks that may arise from illegal and unjustified data sharing and ensuring, in particular, the security of your personal data, the ILO Office for Türkiye reserves its right to request additional documents and information (a copy of your identity certificate or driver’s license, etc.) for purposes of identification and determination of authority. If the information regarding your request under the form is not accurate and current or in the event of an unauthorized application, ILO Türkiye shall decline any responsibility with regard to the requests resulting from such misinformation or unauthorized applications.
